Detective Dave Kelly is still recovering from the horrific injuries he received when attacked by the mutant killer nicknamed 'Ratman'. He thought the terror plaguing his city was over. He was wrong.

A new killer is taunting him with obscure leads and people are going missing at a frightening rate. An enormous pile of human livers discovered in the sewer system under the city and a message written in blood on a bedroom wall tell Kelly all he needs to know and who he needs to call.
