"The clinic is free...but the price is too high!" So says every woman unfortunate enough to be treated at the "Care One Free Clinic." In exchange for "free" medical care, these poor women, young and old, pay dearly by enduring spartan conditions, low-end everything, and a lack of privacy that's downright appalling. Worse, although there is an experienced M.D. running the joint, a certain Dr. Jesse Langford, nearly all of the "free care" offered here is conducted by three horny young college men, including his lonely, touch-starved son, Byron. All are nothing but first-year pre-med students—woefully unqualified to be ordering shivering women out of their clothes and into a plethora of humiliating procedures. What these poor women must endure in order to save money is appalling—all so four dirty young men can have the thrill of their lives. It's disgraceful, disturbing, and disgusting... which, according to these guys, is a very good thing!

In this tale of sleazy injustice, Byron Langford is handed an opportunity not only to order a curvy older woman (Mary) to undress and undergo a very "comprehensive" (i.e., unnecessarily intimate and degrading) physical for a new life insurance policy but to settle a score with her husband, Jim as well. Two years earlier, just after Byron graduated from high school, this man fired Byron for inappropriate conduct after only three weeks of working at his hardware store, thus ruining his entire summer. So now, Byron has a delicious opportunity to settle the score by "requiring" Jim to remain on hand throughout his wife's examination.
