In the aftermath of the Nexus drug war, John Blackwell and his team—Jody, Eleanor, and Mariah—travel to Moscow on a mission to dismantle an interdimensional drug syndicate led by the ruthless crime lord, Dmitri. As they investigate the Nexus operation, they discover that Dmitri has been using the drug to enhance his powers, becoming more than human and posing an even greater threat.

The team is forced into a dangerous game, pretending to cooperate with Dmitri while secretly plotting to bring down his empire. During a series of high-stakes raids, they weaken his network by attacking key drug-processing facilities, but Dmitri retaliates by kidnapping Mariah, forcing John and the team to act quickly.
